About APC
Associates in Primary Care is over 28 years in the making. We take pride in providing state of the art, evidence-based medical care exclusively for adults 18 and older. This means we don't recommend or prescribe things that are not backed by rigorous scientific study. We fully participate with Medicare and most commercial insurances and accept their fee schedules, copays and deductibles as contracted by your payer. Integrity, honesty and transparency are important to us and we always seek to earn your trust. We deliver 'concierge level' service without the concierge price. We just want to provide you with the best care possible.
Services
-
Annual Wellness Visits and Annual Physical Exams - Most insurances cover this service one time a year. It is highly recommended you avail yourself to this opportunity. You and your provider will focus on prevention and screening recommendations, identification of potential medical issues, reviewing your vaccination status, and reviewing your current medications including possible interactions and cost.
-
Acute Medical Issues - Things come up throughout the year and we can handle 90% of acute medical needs in our office or via a Telehealth visit including infections, coughs, colds, mild orthopedic issues, back pain, aches and strains. For serious issues like chest pain, shortness of breath, and neurologic/stroke symptoms, please go directly to the nearest hospital.
-
Men's and Women's Health - PSA's, mammograms (3D or otherwise), bone density, T levels, menopause and hormones are what we do. If you have a gynecologist or urologist, we probably know them and work with them. If not, we can provide the proper care, at the right time, every time.
-
Chronic Diseasc Management - Once you have a diagnosis like diabetes, kidney disease, heart disease, hypertension, obesity, high cholesterol, thyroid problems, COPD, or cancer, we intervene to help manage or reverse the condition by recommending lifestyle modification, medications, and procedures. Then we stand by to monitor or appropriately refer to the best specialist for your condition.
-
Immunizations - APC believes vaccinations save lives and decrease illness. We stock several vaccinations including Tetanus, Flu, and Pneumonia shots. Others can be obtained at your workplace, local pharmacies and health departments. Managing vaccine recommendations can be confusing but we assess your situation, review the CDC recommendations and make recommendations based on your personal situation.
-
Coordination of Care with Specialists, Home Care and Hospitals - Fragmented care can be dangerous. That's why we make every effort to work with your specialists, local hospitals, and pharmacies in order to help manage your care. We have relationships with local hospitals including Beaumont (Corewell), Trinity, Ascension, UofM and Karmanos, as well as talented locally-based specialists. We know the healthcare networks and evolving medical landscape. You can count on us to help guide your decisions.
The APC Difference
-
Personalized Care - Care is optimized when you know and have a lasting relationship with your provider. Episodic care can be good at times, but care over years with someone who knows you in a whole-person context which includes the your medical, personal, social, spiritual and emotional self, is better.
-
Lifestyle-Based Interventions - 70% of medical conditions stem from our lifestyles. Before pills and procedures, we will share things you can do to optimize you health including diet, exercise and weight management.
-
Care Management Services - APC can utilize nurse care managers, nutritionists and social workers when needed. Many of these services are covered by your plan because this team-based approach has been proven to improve patient outcomes while decreasing hospitalization, illness, and overall healthcare cost.
-
Patient Education - An educated individual is our best patient. The more you know about your health and medical conditions you might have, the better and more satisfying your care will be. We want to partner with you in optimizing your health.
-
Measuring Outcomes - We don't just think we are doing a good job, we want to prove it. This is done by measuring our care; we monitor patient outcomes rigorously and regularly.
-
Patient Satisfaction - Another important metric is whether our patients are satisfied with the care we provide. We conduct our own internal assessments but most commercial insurances and Medicare also are conducting surveys. You are encouraged to participate with surveys so we can use that information to improve.
